Case/Care Management | Circles of Care - Outreach Services

The Outreach Services of Circles of Care meets the behavioral needs of children and their families primarily through In-Home and On-Site counseling and case management services. Qualified clinicians who cover the entire Brevard County area provide Children's Targeted Case Management services along with individual, group, and family counseling. Comprehensive wrap-around behavioral services are also provided in residential sites such as those housing children from the Department of Children and Families or the Department of Juvenile Justice and in several homeless shelters in Brevard County. In addition, federal, state, and local grant initiatives are often conducted to further the mission of Circles of Care through our Outreach Services Center.

Circles of Care

Mental health, drug and alcohol abuse, psychiatric care and behavioral health services offered in hospital-based inpatient and outpatient settings. Individual, group and family counseling as well as psychiatric services are available in our outpatient setting Monday through Friday and is staffed by highly qualified therapists and psychiatric providers. Our inpatient crisis units and detox unit serve adults and children dealing with mental health and substance abuse concerns and is open 24 hours per day, 7 days per week. Inpatient units are staff by highly qualified mental health professionals, including psychiatric providers, therapists and mental health nurses.
