Required documents
1. Proof of Idenity: Florida Driver's License, Florida ID card or government issued ID cards.
2. Proof of Residency: Recent mail or electric/water bill with your name and current address.
3. Proof of Child/Children: Birth Certificates and Social Security cards for all children. Birth Certificates must have the name of the person registering for toys.
4. Proof of Need: Current Federal or State Aid, Medicaid, Food Stamps Receipt/Card.
