Eligibility
Current Florida resident over 18 years old Actively undergoing cancer treatment in Florida or within 90 days of your final treatment date. Eligible treatment is chemotherapy, radiation, immunotherapy, hormone therapy, and hematopoietic progenitor cell transplantation Have an annual household income at or below 200% of the Federal Poverty (Will be required to provide bank statements and proof of income for all members of household over 18 years old) Have less than $5,000 in total household liquid assets (checking, savings, CD, stocks etc.)
