Eligibility
Current Florida resident over 18. Actively undergoing cancer treatment in Florida or within 90 days of the final treatment date. Eligible treatments: chemotherapy, radiation, immunotherapy, hormone therapy, and hematopoietic progenitor cell transplantation. Annual household income at or below 200% of the Federal Poverty Level (proof of income and bank statements required for all household members over 18). Total household liquid assets less than $5,000 (including checking, savings, CDs, stocks, etc.).
